I am not familiar with what Fairlane used so it is hard to say if it is correct for that application. Although similar to it is not a style used on a 67 GT500 IMO.

Yes, it is real and is for a '65-'67 427 Fairlane/Comet/Galaxie with closed crankcase emissions because of the elbow.  The curved elbow is the second version, a 90 degree, sharp elbow was the first style, similar to the early GT500 usage.  You can tell it's those applications by the rectangular shaped pad cast into it.  These were never reproduced.
